Description

• Engage in collaborative efforts with nurses, physicians, case managers, and fellow rehabilitation professionals to achieve optimal patient outcomes.
• Guide patients and families in understanding therapeutic objectives, planning for discharge, and performing home exercises.
• Accurately document evaluations, treatment sessions, and patient progress as per facility and regulatory requirements.
• Participate in interdisciplinary care team discussions to facilitate coordinated patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited educational institution.

Certifications

• Valid state license as a Physical Therapist
• CPR certification is preferred.
• Additional specialized certifications (such as neurology or geriatrics) can be beneficial but are not mandatory.
